Multifocal Intraocular Lens Applications (Distance, Intermediate, and Near Vision)

Being able to see every moment, every detail, and all the colors of life freely—without any obstacles—is the most fundamental determinant of our quality of life. However, natural physiological changes that come with aging or the formation of cataracts can restrict this freedom, making us dependent on glasses or contact lenses. A world where you don't search for your glasses to see the clock when you wake up, don't have to lean toward the screen while working, and can watch distant landscapes in full clarity is no longer a dream, thanks to today’s advanced optical technologies.

Commonly known as "Smart Lenses," Multifocal (Distance, Intermediate, Near) Intraocular Lens applications offer a vision of lifelong, continuous, and spectacle-free sight. As Ophthalmology Specialist Ayşe Öner and her clinical team, we bring this revolutionary technology in refractive surgery and cataract operations to our international patients. By blending Turkey’s global strength in health tourism with our JCI-accredited infrastructure and VIP comfort, we promise our patients from all over the world not only clear vision but also a unique, comfortable, and stress-free health travel experience.

What is a Multifocal Intraocular Lens?

In youth, the natural lens inside our eye acts like an autofocus motor. When we look at distance, near, or intermediate ranges, it instantly changes shape (flexes) to focus the image clearly onto the retina. However, after the age of 40-45, this natural lens loses its elasticity (Presbyopia), making reading difficult. In later years, this lens becomes completely opaque, forming a cataract.

Multifocal intraocular lenses are high-tech artificial lenses with special optical rings placed permanently in the eye to replace the natural lens that has lost its function. While "monofocal" lenses used in classic cataract surgeries only provide clear distance vision—requiring the patient to wear reading glasses for books or computers—multifocal (or the latest trifocal) lenses use micro-designed concentric rings to distribute incoming light to different focal points. This allows the eye to clear images at every distance automatically without the need for glasses.

Distance, Intermediate, and Near: Continuous Vision Dynamics

These lenses are engineered to perform flawlessly at the three fundamental distances of daily life:

  1. Near Distance (30-40 cm): This includes fine details within arm's reach, such as reading a book, using a smartphone, checking a menu, or sewing. Multifocal lenses completely eliminate the dependency on reading glasses.
  2. Intermediate Distance (60-80 cm): The most critical distance in modern life. This involves looking at a computer screen, cooking, scanning grocery shelves, or checking the dashboard and navigation while driving. The greatest revolution of new-generation smart lenses is the seamless continuity they create at this middle range.
  3. Distance (1 meter and beyond): The field of vision extending to infinity, such as driving, watching television, playing sports, or enjoying a view. The center of the lens ensures distance vision is crisp and sharp.

Who is a Suitable Candidate for Multifocal Lenses?

"Not every lens fits every eye." A patient's eye structure must be a millimetric match for this technology. Ideal candidates include:

  1. Cataract Patients: Those who want to continue their lives without glasses after surgery and have no underlying diseases in the retina or cornea.
  2. Presbyopia Patients Over 40: Individuals who may not have cataracts yet but are tired of using distance and/or reading glasses and desire a spectacle-free life (Refractive Lens Exchange).
  3. High Myopia, Hyperopia, or Astigmatism: Patients with thin corneas or very high prescriptions who are not suitable for laser eye surgery (LASIK). (For those with astigmatism, "Toric Multifocal" versions are used to correct astigmatism in the same session).

Note: Patients with macular degeneration, advanced glaucoma, diabetic retinopathy, or corneal dystrophy are generally not suitable candidates for lenses that work by splitting light.

The Surgical Process: Painless and Rapid

The application is performed using the PHACO (Phacoemulsification) method, one of the most reliable surgeries in the world:

  1. Drop Anesthesia: No needles or general anesthesia are used. The eye is completely numbed with a few drops.
  2. Micro-Incision: A sutureless tunnel incision of approximately 2.2 mm is made at the edge of the cornea.
  3. Lens Exchange: The natural lens is liquefied using ultrasonic waves and vacuumed out. The permanent, foldable Premium Multifocal Lens is then placed inside the transparent capsule.
  4. Fast Recovery: The procedure takes only 10-15 minutes per eye. Patients are discharged the same day and can typically return to reading and screen use the very next day.

Neuroadaptation: Patients should be aware of the "neuroadaptation" period. Since the brain receives images from multiple focal points simultaneously, it takes a few weeks or months to learn how to select the correct focus. During this time, seeing halos or glare around lights at night is normal and typically diminishes as the brain adjusts.

Frequently asked questions

Yes, this is the greatest promise and success of smart lens technology. These high-level lenses feature intertwined micro-rings (focal areas). When implanted in suitable patients, with or without cataracts, the patient can see clearly at a distance (while driving), intermediate distance (using a computer, chopping in the kitchen), and near (reading a book, looking at a phone) without needing any prescription glasses, as if it were their own natural vision.

Smart lenses are miraculous, but they are not suitable for "every eye and everyone." The biggest secret is selecting the right patient. If you have macular degeneration, glaucoma (eye pressure), advanced diabetic damage, or severe dry eye, a smart lens will bring you dissatisfaction. With the OCT, corneal topography, and aberrometry tests performed when you come to our clinic, your eye's "suitability profile" is determined with 100% clarity. If conditions are suitable, this comfort is offered to you.

Refractive cataract surgery (surgery to get rid of glasses) is the most comfortable face of medical tourism. Your 10-15 minute surgery with drop anesthesia does not require your eye to be bandaged. Often, both eyes are operated on one day apart or in the same session according to the specialist's decision. During this process, you can rest in your luxury hotel in the best locations of Istanbul or Ankara, coordinated by our assistants. The day after surgery, you can experience the freedom of discovering Turkey's historical and cultural beauties with a brand-new, HD, and glasses-free vision.
